To qualify for the $50 Gift Card, you must purchase a new qualifying testo product from any Testo distributor and download the FREE testo Smart App. Offer valid only on purchases of qualifying products made between April 1, 2024 and June 1, 2024, while supplies last and no substitutions or rain checks. Limit one $50 Gift Card per purchase of eligible product. Redemption claim form must be postmarked no later than July 1, 2024. Redemption request must include the fully completed Testo claim form and a copy of the sales receipt of the qualified Testo products. All required information on the claim form must be clearly provided. One claim form is required for each claim requested. Keep copies of materials submitted; originals submitted become vendor’s property and will not be returned. Allow 6-8 weeks for processing of claim. Incomplete or illegible claim forms and claims that do not comply with the terms and conditions of this offer will be rejected and will not be returned. Vendor and/or its agents reserve the right to substantiate claims, and request additional identification and/or documentation. In the event of questions regarding claims, program or policy, vendor will review all documents and make the final determination of claim eligibility.
